One of the exciting new features of the 2011-2012 GO! Diabetes program is the opportunity to participate in the award-winning Diabetes Master Clinician Program of the Florida Academy of Family Physicians Foundation. We currently are recruiting selected private practices and residency programs to apply for the coveted six spots funded by the GO! Diabetes grant. The Diabetes Master Clinician Program includes a diabetes registry, onsite training by Medical Director Dr. Ed Shahady and group visit training.
Change Agents, if you are interested in being considered for participation, please express your interest via email to Susan Reichman, GO! Diabetes Project Director at susan@godiabetes.org no later than February 1, 2012. Please include the names and emails of a lead physician and practice nurse or medical assistant serving as your program champions to improve the quality of your diabetes care.
Dr. Shahady will review the letters and contact each interested private practice or residency program to arrange a telephone interview.
