To start the process, participating family medicine residency programs designate three people as Diabetes Change Agents:
1 – Faculty member
2 - PGY (1) or PGY (2) resident
3 – Clinical patient educator (nurse, psychologist, diabetes educator or pharmacist)

Confirm attendance at a Train the Trainer workshop.

Train the Trainer workshops feature case studies and interactive lectures to provide the most current information on managing the care of the diabetic patient. Diabetes Change Agents receive a copy of the evidence-based curriculum developed by GO! Diabetes lead faculty in PowerPoint format to use in their own residency program training session.

Call for Participation Letter and Application

A few practices will be selected to implement the Florida Academy of Family Physicians’ Master Clinician Diabetes Registry (MCDR), an innovative web-based system producing report cards for both patients and clinicians. The registry helps clinicians and staff to teach and motivate their patients with diabetes during office visits, describe practice achievement of goals compared to other practices in the registry and identify practice patients who are at higher risk for developing diabetes complications.
