Instructions to Register for and Access the METRIC Diabetes:
Improving Patient Care Module for GO! Diabetes 2011-12 Participants
 
 
Before you begin, you will need a valid AAFP username and password to register.  If you do not remember your ID or are not an AAFP member, please call the AAFP Contact Center at 1-800-274-2237.
 
1. Go to www.aafp.org/metric/diabetes
2. Click “Register”
3. Scroll down to the bottom of the page and click “Add To Cart”.
4. Enter your username and password into the login screen that appears.  Click “Enter the Site.”
5. The Shopping Cart should contain the METRIC Diabetes module.
6. Very Important: In the Discount field, enter GO! Diabetes 2011 then click “Apply.” The field is not case sensitive, but you will need to include the spaces. Click “Check Out.”
 
NOTE: Using the discount code grants the Georgia Academy of Family Physicians permission to receive reports on the non-patient-specific data you enter into the METRIC module in order to analyze outcomes.
7. Confirm that the email address is correct, and verify or update the billing and shipping address. You must have a current valid e-mail address entered because METRIC communicates through e-mail. Click “Continue”.
8. Review the information shown.  The unpaid balance should show $0.00.  Click “Submit Order” on the lower right of the screen. You are now registered for the METRIC Diabetes: Improving Patient Care module. You do not need to download anything.
 
Please follow these instructions for beginning the METRIC program:
It is very important to enter your correct group ID in the first METRIC screen.
Failing to do so will mean your data is NOT included in the GO! Diabetes reports.
 
1. Go to www.aafp.org/METRIC. (You may want to bookmark this page in your browser because this is how you enter the program each time.) Click “Begin or continue your module” in the shaded box.
2. Click the Diabetes link.
3. Log in to the METRIC program with your username and password
 
Call the AAFP Contact Center at 1-800-274-2237 if you need assistance logging in.
 
4. Obtain your group ID from your GO! Change Agent or GO! Team Coordinator and enter it in the field “Provider Group ID”. Note: This is the only time you have the opportunity to enter the GO! Diabetes 2011 group to which you were assigned. 
 
5. Press Submit, and you will enter Step 1. You should see your name and group ID just below your own name on the lower left side of Step 1.
