GO! Diabetes and the Video Series

Family physicians account for 25 percent of all medical office visits and treat patients with chronic diseases like diabetes more than any other specialty. These statistics, documented by the United States Department of Health and Human Services, constitute a calling. A calling to know more, do more and achieve better outcomes for patients with diabetes.

The GO! Diabetes program, developed in 2008, heeds the call by providing a streamlined method for evaluating current patient care and achieving better processes by improving patient education and disease management - powerful weapons in the fight against diabetes. GO! Diabetes helps get you battle ready.

In 2010 the GO! Diabetes program offered training to Diabetes Change Agents in residency programs throughout the U.S. and to private practice physicians in Georgia. The video series captured the training workshops that set the course for better managing patients with diabetes and self-assess the results.

The GO! Diabetes video series has been reviewed and is acceptable for up to 5 Prescribed credit(s) by the American Academy of Family Physicians. AAFP accreditation begins September 15, 2010 and the term of approval is for one year from this date with option for yearly renewal.

Part I: Better Disease Management 1.0 Prescribed credit
Part II: Detection and Lifestyle Management .50 Prescribed credit
Part III: Diabetes Educator 1.0 Prescribed credit
Part IV: Glycemic Control –Oral Agents 1.0 Prescribed credit
Part V: Glycemic Control – Injectibles .75 Prescribed credit
Part IV: Cardiovascular Disease .75 Prescribed credit

Simply click on the above links to view the videos. At the completion of each video vignette, please click on the Evaluate and Request CME link displayed beneath the video to complete the evaluation. Once complete, please fax your evaluation to the GAFP to receive your CME.
