As you heard at your Train the Trainer session, this year we added a new activity to our GO! Diabetes project, the “GO! Diabetes Patient Experience Study.”
 
The goals of this study are to:
1) Improve providers’ communication skills with patients
2) Enhance patients’ knowledge about Type II Diabetes and their self-management skills
3) Assess and improve the patient care experience
 
This study will be designed and managed by Integrated Learning Partners (ILP; a collaborative educational partner in this year’s grant. ILP specializes in performance optimization, educational research and integrated learning for clinicians and their patients.
 
We are looking for six practices to participate in this innovative project. Each participating practice will receive a stipend in the amount of $1,000.00.*   The requirements to participate are:
 
• As a practice, you must select three (3) providers (e.g., MD, DO, NP, PA and/or RN) to participate. Each provider will need to complete two 15-minute surveys (a baseline and follow-up survey), a 1-hour online CME activity, and use elements of the clinician-patient toolkit when dialoguing with patients about their diabetes management plan.
• Each participating provider must randomly select ten (10) eligible patients for enrollment in the study. Office staff will be responsible for distributing patient enrollment postcards to (10) type II diabetes patients. The first five patients per provider that respond and return their agreement cards will be selected to participate.
• Patients who return their agreement postcards and opt to participate in this study will also be asked to complete  - and post-intervention survey. They will receive a $10 gift card after completing the first survey and a $20 gift card after the second survey.
 
*Note: Your practice will receive $1,000 if you enroll three providers and 15 patients; otherwise stipends will be prorated.
 
This project is HIPAA compliant. All patient data collected will be anonymous, and aggregate data reports will be presented at the conclusion of this project.  The total time commitment for each provider is projected to be about 3.5-hours over the next six months.
